Of course it can assess anyone but the most important request was, "I am looking for a simple and customize-able low-literacy biometric screening sheet describing the following risk factors with a field to enter each reading....(list)."
We talked to our graphic designer and she created this form.
It allows people to enter all of the most important data to help them assess their current health and start improving the areas that need help. It is like a "health-at-a-glance" sheet that depends on basic biometric measurements. A Biometric Screening is simply a tool which provides a clinical assessment of key health measures.
