Siobhan Donegan, MS, RD, Vermont Dept. of Health, requested this form to assess women in its women's health program. Of course, it can assess anyone, but the most important request was, "I am looking for a simple and customizable low-literacy biometric screening sheet describing the following risk factors with a field to enter each reading....(list)."

We talked to our graphic designer, and she created this form.

It allows people to enter all of the most important data to help them assess their current health and start improving the areas that need help. It is like a "health-at-a-glance" sheet that depends on basic biometric measurements. A Biometric Screening is simply a tool that provides a clinical assessment of key health measures.

5. Blood Cholesterol including HDL, LDL, total and triglycerides

Best of all, there is a little blank spot at the bottom left of this PDF. What can you put in that spot? Why not handwritten notes to the attendee, your logo, or some other important message? You can have a graphic artist or anyone good with Adobe Acrobat Pro or Adobe Photoshop to enter the logo or message in the PDF and then print yourself or have it printed.
