Biometrics Screening Tool Handout Tearpad - Health Fair - Corporate Wellness and Screening

Siobhan Donegan, MS, RD, Vermont Dept of Health requested this form so she could use it to assess women in their women's health program. Of course it can assess anyone but the most important request was, "I am looking for a simple and customize-able low-literacy biometric screening sheet describing the following risk factors with a field to enter each reading....(list)."

We talked to our graphic designer and she created this form.

It allows people to enter all of the most important data to help them assess their current health and start improving the areas that need help. It is like a "health-at-a-glance" sheet that depends on basic biometric measurements. A Biometric Screening is simply a tool which provides a clinical assessment of key health measures.

1. BMI
2. Waist Circumference
3. A1C
4. Blood Glucose
5. Blood Cholesterol including HDL, LDL, total and triglycerides
6. Blood Pressure

The back side to this tearpad is a glossary of terms. There is a blank spot on the bottom left that can be used for custom instructions or handwritten goals.
